By using the services of Care Counseling & Children's Services ("we," "our," or "us"), you agree to comply with these Terms and Conditions. If you do not agree, please do not use our services.
2. Services Provided
We offer:
- Psychotherapy (individual, group, or family therapy)
- Medication management (evaluation, prescription, and monitoring)
All services are provided by licensed professionals.
3. Appointments & Cancellations
- Appointments can be scheduled via phone, email, or our patient portal
- Cancellations require at least 24 hours' notice to avoid a late cancellation fee
- No-shows may result in a fee and possible discharge from the clinic
4. Fees & Payments
- Payment is due at the time of service unless otherwise arranged
- We accept cash, credit cards, and most major insurance plans
- Unpaid balances may result in suspension of services
5. Confidentiality
Your privacy is protected under HIPAA and applicable laws.
Exceptions to confidentiality include:
- Risk of harm to yourself or others
- Suspected abuse or neglect
- Court-ordered disclosures
6. Patient Responsibilities
- Provide accurate medical and personal information
- Attend scheduled appointments and follow treatment plans
- Notify us of any changes in contact or insurance details
7. Limitation of Liability
We are not liable for any indirect, incidental, or consequential damages arising from treatment.
In case of emergencies, call 911 or go to the nearest emergency room.
8. Termination of Services
We reserve the right to terminate services if:
- There is a violation of clinic policies
- Non-payment of fees
- Threatening or abusive behavior toward staff
9. Changes to Terms
We may update these Terms and Conditions. Continued use of our services constitutes acceptance of changes.
